This results in a current health care system that can be difficult to navigate, lacks patient-centeredness and can be problematic to coordinate care. 93% of Americans want one place or doctor that provides family care and coordinates care with specialists. However, only half report having such an experience (Stremikis, Schoen, and Fryer, 2011).   The patient-centered medical home (PCMH) aims to rebuild primary care and achieve better quality, improved experience, and lower costs.

The medical home concept began in the 1960s as a way of improving care for children with special needs. Having all of the physician services under one roof made the practice of going to the physician easier, rather than driving to many different locations for a multitude of medical ailments. Intrigued by the potential of the medical home model, private insurers and State Medicaid and Medicare agencies began testing the concept.

Lifecare Family Health and Dental Center is a patient-centered medical home that has been serving our community since 1994. We improve the quality of care by providing comprehensive health and support services to meet all of your healthcare needs.  This comprehensive care requires a team of providers. Our team of health care professionals includes family physicians, nurse practitioners, dentists, dental hygienists, optometrists, behavioral health counselors, psychiatric nurse practitioners, health educators, and a podiatrist.

Lifecare Family Health and Dental Center also provides affordable and accessible care by being centrally located in areas that are medically underserved, and payments based on the patient’s ability to pay using a sliding fee scale. Private insurance and self-pay are also accepted.
